Please review and be prepared to sign the Pool Usage Agreement form at the pool if you wish to use the pool.
POOL USAGE AGREEMENT DELRAY OAKS WEST
Pool hours: 9:00 AM to 11 AM for Physical Therapy only. Noon to 3 PM All Others. Hours are subject to change. Restrooms are to remain closed.
No more than twenty-nine (29) people at one time. This allows for social distancing on the deck and in the pool. Children under eighteen must be accompanied by an adult who will enforce CDC rules.
▪ No food or glass bottles. Two (2) chairs per table at all times. ▪ Lounge chairs six (6) feet apart.
▪ Wipe chairs with disinfectant upon arrival.
▪ Wipe hands after touching gate.
▪ Residents only. NO GUESTS.
▪ Maintain a safe distance at all times, in and out of pool, except with family members. ▪ Wear mask when not swimming.
▪ Wash hands upon returning to your unit.
▪ Report violations.
I, _________________________________________ AGREE TO ALL OF THE TERMS OF THIS AGREEMENT AND WILL ABIDE BY THEM. IF I FAIL TO ABIDE BY THESE RULES I MUST LEAVE THE POOL AREA.
